Change of Address Form

In accordance with the Nevada Administrative Code, 623A.150: "Submission of business address required, change of address...."

1. A registrant shall file his proper and current business address with the Board.
2. A registrant shall notify the Board in writing within 30 days after any changes of address.

If you have any change of demographic information, please complete the form. The notification must include both old and new address.
